    My Experience:My two first SP`s were a woman without her son and a phone call from a mother of a youg child. I knocked on the door/ picked up the phone introduced myself and started to ask how I could help them. I was a little bit unsure what to asked them about their sons who weren`t there..so I did my best and ask about achieving developmental milestones, and the current problem, history and so on. I gave both the recommendation to bring their sons into the clinic/ER b/c of futher evaluation/ possible life threatening condition. Both seemed satisfied. Next SP c/o acute SOB. SP was in acute distress. Tried to give him oxygen via mask. Paitent was uncomfortable with this. I didn`t insist and started to ask questions about his condition and did a quick exam. I didn`t gave recommedation to stop smoking...question of the SP if he has to stay in the hostpital: told him brief he has to stay here a least one day and will be monitored. He will be treated and released when his condition has improved. The next SP c/o back pain. I asked SIQOORAAA + PAMHRFOSS. Did a quick exam of the spine/ DTR`s/ Spine movment/ Gait. Forgot Lungs and Heart...SP aked for pain medication. I told the SP that I would like to do the exam before giving the pain medication. But as soon as I am finish I will obtain some...so at the end of the case I told the SP to be back in a few minutes and bring the pain medication. Next SP was a man with urinary problems. I asked SIQUOORAAA/PAMHRFOSS. In order to understand his condition a little bit better I asked him to give me some more information about his sexual history. Told him that this might be a problem with the prostate which is a commen problem in men at his age. In most of the cases it is treatable and told him to do futher test including rectal exam. SP seemed satisfied. Next SP c/o weight loss and fatigue. I asked SIQUOORAAA/PAMHRFOSS. No FHX/o cancer. I told him that this condition coud be due to a process in his stomach. Before I can give him more information we have to do futher tests. Explained him the upper endoscopy... Next SP had sore throat. Asked SIQUOORAAA/PAMHRFOSS. Asked STD`s. I didn`t gave him recommendation to use condoms...Question of the SP was if he can have some antibiotics and can leave the clinic: I told him that we have to do some more further tests and then he`ll might get some antibiotics but has to stay in the hostpital as long as we´re not sure about his condition. Didn`t gave him advice to be careful with sport or exercise since he could have EBV. Next SP had an episode of VT no faint. Aked her SIQUOORAAA/PAMHRFOSS. Family History of sudden death unknown origin. Didin`t empathize so much b/c of time issues and I wasn`t aware that this component count so much. Told her that we would do a 24/h EKG. Explained EKG to SP. Next SP c/o fatigue, asked her SIQUOORAAA/PAMHRFOSS. No specific history or complaints. My suspision was Hypothyreosis. Did the neck exam/ Neuro exam. Told her that we have to do some more futher test regarding her thyroid and we`ll do some blood tests. Can`t remember the rest of the SP´s. Sorry. I asked every SP at the end wether he or she`s got any question for me. Everyone denied. So I left the room by saying I´ll prepare for the tests and will be back in a few minutes.
    My Advice & My Errors:My performance: Passed in spoken english (high perfomance), passed in Patient evaluation and Data gathering (middle to borderline performance), failed in communication and interpersonal scills. I wasn`t smiling all the time b/c I thought there was nothing funny about the SP`s conditions. I thought I empathize whith being serious with the SP`s - maybe I was wrong...I didn`t gave recommendation in special cases. Otherwise I´m a little bit confused about this outcome. I knew I wasn`t good, but I thought I would pass... All in all I think I missed to talk about personal and social issues with the SP´s. I tried to write down the cases as good as I remember them, so maybe you can figure out what the problem might have was. Next time I would do the Kaplan Prep b/c I am not trained in the US.
